LIABILITY WAIVER:

I acknowledge that participation in Hoop Lab activities involves inherent risks, and injuries may occur. While rare, these injuries can sometimes be serious. In consideration of my child’s participation in Hoop Lab activities, I, the parent/guardian, assume all risks of injury and agree not to hold Hoop Lab, its directors, coaches, assistants, agents, or volunteers liable for any injuries that may arise from participation. By signing this waiver, I also grant permission for photos or videos of my child to be used for website or advertising purposes by Hoop Lab.

IMAGE RELEASE:

As the parent/guardian (or player, if 18 years or older), I hereby give Hoop Lab permission to use my or my child’s likeness in photographs, video recordings, or other electronic images for publications, including website entries and social media, without compensation. I understand that these materials will become the property of Hoop Lab and will not be returned. I also grant the right to edit, copy, publish, or distribute these images for marketing or promotional purposes, waiving the right to inspect or approve the final product.

CANCELLATION POLICY:

If you wish to cancel a Hoop Lab membership, notice must be provided before your next billing date. Failure to notify before the billing date will result in your account being charged. Refunds will not be issued for cancellations that are not submitted in advance.

TRAINING GUARANTEE:

Hoop Lab does not guarantee a specific number of training sessions per week or month, nor a specific duration for each session. Changes to the advertised training schedule may occur as needed.
